Outreach provides 1 to 1 support
to a carers & families to help them emotionally and practically
throught a very lonely and confusing time.
These services are available
to brain injured people, their families and/or their carers.
Outreach Workers can provide a service at any stage of the
brain injury pathway and are able to visit clients at hospital,
in their homes or at a Headway site.
The team is limited to the
distance they can travel and usually visit within the Birmingham
& Solihull Areas, though we will do our best to help those
outside these areas.
The Outreach
Worker will arrange a visit to make an initial assessment of
the families/carers needs. Visits can be either to the
home, at hospital or at a building of choice.
The following
services may then be offered:-
· Support which
may be emotional — in
the form of a friendly listening ear — or practical - providing
information on brain injury, advice or guidance. The worker
will try to answer some of the many frequently asked questions
to help you understand more.
· Assistance
in accessing other relevant available services (e.g. medical, financial assistance,
other brain injury services, social services
etc)
· Advocacy— Outreach
Workers understand
the difficulties and complexities of the effects of acquired
brain injury. They are therefore able to support, and/or
act as a representative of their client with empathy
and knowledge when dealing with other professionals, in court,
at benefit appeals etc.
Following
assessment, there will be an agreement of the tasks that the
Outreach worker will try to do for the client. When
the worker has done all they can do to complete the tasks, the
file will be closed. The
client can access the Carers
Support & Training
services at the same
time and continue to use these after closure with Outreach.
Outreach can also be provided as
part of the BMEG
services, with the extra ethnic related support as required.
Outreach is short-term help, but
a client case can be re-activated at any time should the family
need further assistance.
